In case of family medical emergencies, it makes sense to have this data at home too, so print it out and take it with you! In the appendix you will find the most important telephone numbers that you could use for medical emergencies, please separate this appendix and keep it in your papers! This report can also be made available to employees of the German School and other German organizations on site, please forward it to the GIZ office! It is not desirable to pass this information on to commercial users (e.g. insurance companies) or to post it on the Internet. The evaluation of the medical care structures by the regional doctor serves to demonstrate good, adequate medical care in the event of illnesses, injuries and life-threatening emergencies of posted employees and local staff of the Foreign Office, but also of German, European and other posted foreigners as well as tourists in Beijing and beyond to ensure the employer's ability to provide information. The evaluation is limited to medical facilities in Beijing. For information on the general living situation of the posted employees, the embassy report on living conditions is available on the intranet as well as the constantly updated travel and safety information on the homepage of the Federal Foreign Office. The medical infrastructure in Beijing is not bad, there are good hospitals for international patients as well as outpatient meds for the Chinese population. Care is assured, also for international patients, there are several modern, well-equipped clinics available. The medical training of the Chinese colleagues is difficult for the outsider to evaluate. For this reason, posted employees should continue to seek help in their home countries with serious medical problems, not least because of language difficulties! It's worth it: a really modern, spacious dental practice, a generous welcome, very friendly staff. Dr. Metzmacher is a very pleasant colleague and makes an excellent professional impression. Here everything can be done dentally, for you and your children. At a high professional level and ---- in German. There are also good Chinese dentists here, they speak English, surely a good substitute for Dr. Butcher. Dental implants can also be used in the same quality as in Germany. Prepared for expats and wealthy Chinese too. About 20 doctors are employed, a total of about 200 employees. Dr. He, Medical Director, was in Nancy, France, for specialist training. She is fluent in French and English. All common ophthalmic examinations and treatments are carried out here, the device is modern, and the medical colleagues make a competent impression. Acute ophthalmic problems, simple eye tests with prescription of glasses are possible, even complicated treatments, including chronic eye diseases, are carried out at an internationally high level. Attention - sometimes TCM approaches are difficult to understand for Western Europeans! A treatment lasts approx. 40 minutes. Tension caused by strenuous office work is also treated. Good English speaking, friendly doctor who holistically performs TCM in connection with Western medicine. It is particularly helpful with orthopedic problems. Medical evacuations There are 5 SOS clinics in China (Beijing, Shenzhen, Nanjing and 2 x in Tianjin - Tianjin and Tianjin TEDA). Comments International-SOS in Beijing has some competent (and very nice), German-speaking general practitioners and physiotherapists. The clinic is conveniently located to the German Embassy and to some residential areas of the embassy staff. Emergency medicine is one of the main focuses of the clinic, they are well equipped and have technically competent staff. Medical assistance (for members) throughout China (but also worldwide) is possible, several Chinese doctors provide competent med. Centers all over China, if the patient does not speak Chinese enough, there is language assistance! International SOS is one of the most expensive med. Facilities for expats in Beijing, but the fees are covered by most German private health insurances! (Members of the German Embassy receive a discount, they pay like SOS insured persons). In the BUF-Hospital and also in the other, external practices, general medicine is of course also offered, the rooms are very attractive, everything is hygienically sparkling clean! In the hospital, emergencies can also be taken care of, a children's consultation hour for emergencies is set up 24 hours a day, 7 days a week. There is a department for integrative medicine, where TCM and Western medicine methods are used together. Pregnant women are cared for at a high professional level, normal deliveries can be carried out well here! Outpatient and inpatient in the BUF hospital (in Jiagtai Lu) Outpatient in the above-mentioned satellites, there are also some specialists who can be consulted! 17 Diagnostics Laboratory X-ray = imaging examinations Therapy Very decent laboratory with modern laboratory computers for blood chemistry, blood cell counters, antibody diagnostics. External quality controls are carried out 3 to 4 times a year (own information). Blood chemistry tests are carried out from approx. 100 blood samples and blood counts from approx. Automatic urine test readers. It maintains its own blood bank, and the blood supplies come from a central government agency. Very modern X-ray machines e.g. Magnetic Resonance Tomography (GE 1.5 Tesla); modern CT (Siemens - 64 slices). Contrast agent examinations are also possible (auto-injector) mammography device. Cardiac catheter laboratory so far only a few examinations are carried out per month (approx. 3 4). Renowned Chinese cardiologists work here (Dr. Hu Dayi, Dr. Ren Wenlin, Dr. Bai Shuling). Bypass operations and heart valve replacements have not yet been performed, a license has been applied for and trial operations have taken place. Pediatrics 6 neat, modern examination rooms. Maternity ward Pregnancy care, delivery and post-delivery care are an important part of UFH's business. Very nice and functional circuit rooms. The UFB prides itself on having the lowest number of caesarean deliveries in Beijing (approx. 33%), which would also correspond to German figures. Approx. 150 deliveries are carried out per month (!!) Large pharmacy. Here, certified med. Chinese production but also imported medication from Europe are sold, and prescriptions from the German-French practice are redeemed. Emergency room with a separate entrance and driveway for the ambulance. 15 examination boxes (with negative pressure!), Patient stretcher. Modern equipment. Not only real med. Emergencies treated but also normal illnesses outside of office hours. There would be 6 mod. Operating theaters, I haven't seen any! Endoscopies would be done. Dentistry several modern treatment rooms. Dentists who have been trained or trained abroad (USA, Canada). There are dental hygienists and dentists who specialize in children, and orthodontics (braces) are offered. All dental med. Procedures including implantology are done. Very well trained dentists, English and French are spoken. Hospital beds 120 very modern beds in single rooms, 70 beds are occupied. Everything necessary for the monitoring device is available here. The stations are around 3 years old, and Chinese and European cooking is done. Station for integrative therapy here, Far Eastern and Western medicine is combined for certain clinical pictures. Hygiene very good! The hospital is ISO standard certified! 17 medical departments are represented by their own doctors; specialists still come to the hospital by the hour as consultants. A total of around 120 doctors work in Beijing United. Many doctors received training or further training abroad. Actually all illnesses can be treated here, only severe trauma and burns are given to other (Chinese) hospitals. Very attractive private hospital with first-class hotel flair! 18 The gynecological and obstetric department is very nice and could also be used by expat women. Many German women give birth here, luxurious service is offered, and there is good pediatric care for small children. High-risk pregnancies should rather be carried out outside the host country, e.g. give birth in Germany. A word about child vaccinations as in other large private clinics in Beijing there are no modern multiple vaccines like in Europe! Child vaccinations should preferably be done at home! There is a trustworthy emergency admission station. The Liangma Clinic has just been modernized and enlarged near the German School and the Kempinski Hotel. This is Dr. R. Diagnostics Laboratory X-ray = imaging examinations Therapy Very modern diagnostic equipment available in the laboratory and X-ray (see below) Well-experienced European trained but also Chinese staff (doctors and paramedics), (almost) all of them speak English (more or less understandable). Very tidy laboratory with modern laboratory computers for blood chemistry, blood cell counters, antibody diagnostics. The extent to which international quality standards are complied with has not (yet) been found out. Very modern X-ray machines e.g. Magnetic resonance tomography (Siemens 1.5 Tesla); decent CT (from GT, 16 slices). Modern digital Rö device (Chinese design) for bones and lungs. (Unclear who is evaluating the pictures). Ultrasound with Chinese devices, the examiner actually spoke no English (exception?) Gynecology and Obstetrics (Dr. Stanford trained in the USA). So far, no emergency situations have occurred during childbirth. Normal pregnancies are better looked after here than in Colorado. Only 21% of births are delivered by caesarean section. Approximately 20 deliveries per month are carried out. Very nice individual circle rooms, very decent mother-and-child ward (similar to a hotel!) 20

21 Very good hygiene! Laparoscopic examinations and operations would be performed. No IVF therapies (= artificial inseminations) in Oasis. The intensive care unit cannot be recognized as such. The beds (5 + 1 isolation room) are in single rooms, the patients are monitored by monitors, the nurses are in front of the rooms (there were no patients during the visit). Emergency room with a separate entrance and driveway for the ambulance. 2 very neat tri-bed beds. International emergency medical equipment available. Infusion pumps, ventilators, monitoring monitors. The nurses are stationed in front of these beds (e.g. Jane, Chinese sister, speaks excellent English). The recording is here at night. Very nice, amiable colleague (she is currently doing your specialist in UK) in a very nice children's ward with a sick clinic (for acutely ill children) and a well clinic (for preventive examinations and vaccinations (separate entrances, of course). There would be 3 operating theaters - Halls, I haven't seen any! Endoscopies would be performed. TCM with at least 2 doctors. TCM would be particularly effective for heart diseases, muscle pain, but also for colds. Dentist several modern treatment chairs, doctors from the USA (Dr. David Lee) and from Taiwan (Dr. Grace Lee). Implantology also possible. Physiotherapy (Jason Tam and Jeff Cauver French, chiropractor) a neat, small facility with fitness equipment. Exercises to treat poor posture pain when using a PC are also carried out. Ophthalmologist comes all day (more on Glasses prescription); ENT doctor - comes twice a week. Comments Very attractive private hospital with first-class hotel flair! For employees ith the embassy and its family members, a free membership to Oasis can be entered into, the hospital costs are then reduced by 20% (discount)! 33 3. Health risks In the giant country China there are numerous medical problems, including some that we do not have in Germany or not to the same extent. The chaotic traffic, at least in the big cities, has the potential to generate car accidents. The sometimes harsh climate here can increasingly lead to colds, the tempting food on the streets takes its toll, especially in summer, the diarrheal diseases are becoming more common. You will not believe there is no prostitution in China and sexually transmitted diseases do occur, not that rare. Fortunately, HIV / AIDS is rarely more than that afterwards. The health hazards caused by air pollution (smog) and the deadly danger of infection with rare bird flu viruses are a source of excitement. Medical care in Beijing is not bad in the outpatient as well as in the inpatient area, at least for non-complicated illnesses! Not only the air has pollution problems, also the water, which is used for irrigation in the really dry environment of Beijing. River water around Beijing certainly contains heavy metals and pesticides, the only question is in what concentration? The big supermarkets here seem to be randomly investigating pollution in vegetables !? And what is the offer on the markets? That is better? Where are these farmers supposed to get clean water from? Fertilizers and pesticides are sure to be used by them too. The fish offered here also mostly comes from fish ponds with the same problems. There are certainly more dangerous foods - such as: b. Fast food consumed in excess or too much sweets anyway: be careful, especially with baby food (melamine scandal 2008) !! Frozen sea fish from Norway is probably safer, and imported baby food may also be. 33

34 c) Health hazards from smog: As you know, Beijing has big problems with air pollution. This depends on special environmental factors. It is important to know that Beijing is smog-free most days of the year! Smog days are common, but luckily the beautiful days with blue skies and sunshine in winter and summer predominate. Smog is usually a mix of natural pollutants (fine dust from the Gobi desert), pollutants emitted by cars and industrial plants, or by burning, cheap, pollutant-rich coal (hence the pollution peaks in winter). The pollutants include fine dust (P.M. <2.5), but also nitrogen oxides (NOx), ozone, sulfur dioxide (SO2). High concentrations in the air and prolonged exposure times can lead to health problems in humans (respiratory tract asthma, chronic lung diseases such as COPD; but also cardiovascular damage). Infants and toddlers as well as old people or those with previous illnesses (chronic asthma, known COPD) are particularly at risk. Internationally, ultra-fine particulate matter (PM <2.5) is regarded as the main substance of air pollution. In Beijing and in other affected metropolises in China, this value is measured regularly (hourly) by automatic measuring stations, the values ​​can be accessed online on the Internet (app), including values ​​that are determined by the US embassy in Beijing. The measured value is the Air Quality Index (AQI), in which the fine dust concentration is significantly included. Although we use other measured values ​​as a basis in Germany - only measure the fine dust concentration PM <2.5 and in mg / l (air), the AQI is comparable with our measured values ​​(especially at low concentrations). The fine dust concentrations (= approx. AQI) should not exceed values ​​of 25 mg / L (air) per day, the annual average should be below 50 mg / L (air). We here in Beijing are actually only starting to think about health problems at AQI values ​​of 100. And --- Beijing is only 10th in the national smog ranking in China! What does it all mean in concrete terms? People with severe asthma that is difficult to treat should not be transferred to Beijing. Also not patients with a severe, poorly treatable chronic obstructive pulmonary disease (e.g. ex-smokers). Patients with severe, poorly treatable heart diseases (heart failure, arrhythmias) should also be careful with smog duty stations, they do not belong here! What about children, infants and toddlers? There are also a lot of Chinese children in Beijing who grow up healthy despite the smog! For healthy infants and children, there are certainly health risks from smog (increased risk). However, we doctors believe that this is reasonable in terms of health for this clientele if they are only exposed for a certain period of time (e.g. 3 to 4 years) and minimize exposure by staying in clean air areas. Increased health disorders (compared to non-exposed control groups) can then no longer be reliably detected! The risk of suffering serious damage to health from not wearing a bicycle helmet is certainly significantly higher! What can you do, what is important? .. smog is unhealthy! If it occurs, there are precautionary measures that should be observed (see the recommendations of the US embassy in Fig.). You should avoid smog when it is there! Your workplace but also your apartment should be equipped with air purifiers, the pollutants are filtered within a short time, the air is clean! It works, it was measured !. If you do after 34

If you have to go outside, masks and dust filters are used (e.g. SSP 2 masks, which e.g. the German embassy provide their employees with free of charge). These dust filters have been proven to do what they should, they significantly reduce fine dust in the air we breathe! There are also dust masks for children, probably only in China, but here they are e.g. sold in the international clinics - hospitals, they work too! When things get really tough and stay a few days, why not take a short vacation by the sea! 35

36 d) tetanus, diphtheria, polio, whooping cough. These are diseases that occur worldwide, including in China. Regarding polio: If you are traveling to China from Afghanistan, Pakistan, Iraq or Nigeria, Cameroon, Equatorial Guinea, Ethiopia, Somalia or Syria, a polio vaccination certificate that is not older than 1 year will be required! About whooping cough: Did you know that around 20% of persistent (= chronic) coughs are caused by whooping cough pathogens? There is a combination vaccine that contains all 4 antigens and should be refreshed every 10 years in adults. e) Infectious jaundice (= hepatitis A) This infectious disease is easily transmitted, the finger of an infected person in the soup is enough to infect the guest !! It is common in SE Asia, cases have just been reported in Taiwan. People over the age of 60 can die of hepatitis A! Make sure you are vaccinated! The vaccination protects 100% and has almost no side effects. After the primary immunization (injections at least 6 months apart), booster sessions are recommended every year, I sometimes think, maybe it will even protect you for life. f) Rabies = Rabies Exterminated in Germany for almost 10 years (you only have to think about it when you come into contact with bats) Rabies is a huge problem in East Asia, almost people die of this disease in China every year, here it is mostly still caused by transmitted to rabid dogs. Rabies occurs everywhere in the country, but especially in the south and southwest. The vaccination protects you safely against rabies (vaccinations on days 0, 7, 28 or later). Refreshments are recommended after 5 years. g) Typhus (typhus abdominalis): Typhus is one of the classic epidemics, but fortunately only rarely occurs. In Germany there are fewer than 100 cases that are introduced each year. Typhoid is a disease of poor hygiene. When traveling in China in such conditions, infection is possible. The causative agent of typhus is Salmonella typhi. There is a vaccination that only protects about 2/3 of those vaccinated and only against real typhoid. The typhoid vaccination does not prevent diarrhea caused by other salmonella. h) Japanese encephalitis: This virus infection, which can be transmitted to humans by house mosquitoes (Culex), also occurs in southern China and on the coast (in the warm season). The virus can survive in pigs and waterfowl (ducks, herons) and then be transmitted to humans by mosquitoes (which suckle on birds, pigs and humans, e.g. Culex). The risk of infection is therefore seasonal (May to October) and more in the rural coastal area. 36

37 There are high case numbers in the following provinces - Guizhou, Shaanxi, Sichuan and Yunnan, Chongquin City. Since 2008, there have been around approx. Local children in particular are victims of this serious viral infection, which only breaks out in some of those infected. In the sick Japanese encephalitis heals in about 1/3 of the cases, in a further 1/3 there is defect healing in the brain and about 1/3 of the sick die from this infection. Japanese encephalitis is a very rare risk for tourists, there is a very well tolerated vaccination that protects safely (see below) i) Dengue virus infections: There is a low dengue risk in China! It is transmitted to humans by large mosquitoes (Aedes) and causes a very unpleasant fever, it comes (if it does) as an endemic. Particularly in rural regions of the southern provinces of Guangdong, Guangxi and Hainan are affected. No dengue risk in Beijing and Shanghai! The vector mosquitoes are diurnal, repellants (in case of danger) should be applied during the day! j) Early Summer Meningo Encephalitis (TBE) or Russian Spring Summer Encephalitis (RSSE): A disease that can be transmitted from ticks to humans, which also occurs in southern Germany, Austria and Switzerland. A longer camping stay (hiking) in rural areas of the northern or north-eastern provinces (border with Russia April to October) should make you think about vaccination. These provinces include: Jilin, Heilongjian, and northern Nei Monggol. The TBE vaccine available in Germany is very effective.k) Schistosomiasis (= schistosomiasis): This rare parasitic disease also occurs in freshwater rivers and lakes in China and Africa! The infectious larvae can penetrate the intact skin on contact with fresh water and lead to disease (chronic inflammation, bloody urine). The following endemic zones are known: Dongting and Poyang Lake, the Yangtze River with the surrounding marshland in Hunan, Jiangxi, Anhui, Hubei and Jiangsu. But broadcasts are also possible in more mountainous regions such as Sichuan and Yunnan. The risk is low. Despite being careful when coming into contact with fresh water in these regions! If you were exposed or had contact with fresh water there, a schistosomiasis test could be done after leaving China. Therapy for the disease is easy! l) Avian influenza H7N9 (but also H5N1): This avian influenza pathogen was first detected in China in 2013, it did not leave the country until January 2015 (2 cases in Canada). Birds, especially chickens and ducks, can become infected with H7N9, but they do not get sick (with H5N1 the poultry get sick too!). If people live closely with these animals (e.g. chicken farmers) or if they come into close contact with infected chickens, the virus can spread to humans. If avian influenza H7N9 occurs in humans, this virus can lead to a very serious illness; death rates of up to 30% (of the cases that have become clinical) have been described (the infected but not sick have not yet been recorded; the cases from Canada were mild Gradients). 37

38 What can you do yourself to avoid being endangered by bird flu viruses? Do not visit chicken farms, eat chicken and duck meat that is only very well cooked! If such visits are necessary for business purposes, breathing masks (FFP 3) should be worn urgently! A transmission of this virus from person to person has not yet been proven! m) Malaria: You won't believe it, but in the summer (May to December, below altitude) there can be malaria infections in China, as you know, a mosquito-borne infectious disease. One (CDC = Center of Disease-Control and Prevention, Atlanta, USA-2014) assumes that the benign form of malaria (Plasmodium vivax) predominates, but the dangerous form (Pl. Falciparum) also occurs (rare, border with Myanmar, Hainan Yannan Province). The risk of infection for travelers is rather low. The risk is rather low and more to be expected in the southern parts of the country. If you have been there and develop a high fever within 3 months, you should also keep this rare disease in mind. When you are there, use mosquito sprays (repellants e.g. Autan, Nobite), sleep under mosquito nets or in air-conditioned rooms! n) Sexually transmitted diseases, HIV, hepatitis B: These are not uncommon among tourists in China. Of course, it is unclear what the reason for this could be, because prostitution does not exist, as is well known. Compared to Africa, HIV / AIDS is much rarer, but compared to Germany, it is at least 10 times higher! However, like in Germany, the main transmission route for HIV in China seems to be intravenous drug consumption (needle sharing). Hepatitis B is also one of the sexually transmitted diseases, there is a very safe vaccination protection. In order to avoid the other undesirable infections, sexual contact should only take place with condoms, especially if you have not known your partner that long. For long-term or work visas, a med.
